PULMONARY OEDEMA FOLLOWING TOPICAL PHENYLEPHRINE ADMINISTRATION IN A CHILD ANAESTHETISED FOR CATARACT EXTRACTION. (28 Apr 2008) Ophthalmologists commonly use locally applied mydriatic drugs perioperatively. Topical phenylephrine solutions are widely used both for capillary decongestion and for papillary dilatation.1 Phenylephrine is predominantly an á-adrenoreceptor agonist and significant systemic absorption can occur after topical administration which can lead to adverse cardiovascular consequences including hypertensive crisis,2,3 tachycardia, reflex bradycardia, ventricular arrhythmias,3 myocardial infarction and cardiac arrest.4 We describe a case of acute pulmonary oedema following topical phenylephrine administration in a child anaesthetized for cataract extraction.
Prevalence and Risk Factors of the Rural People on Prehypertension Status in Lanxi County of Heilongjiang Province (23 Apr 2008) Background: To access the prevalence of prehypertension and associated risk factors in residents of rural Lanxi county in Heilongjiang province.
Methods: Through cluster sampling methods, a resident group of 5272 residents aged ¡Ý15 years old in Lanxi county was selected. A survey on blood pressure (BP) and associated risk factors was carried out. An overnight fasting blood specimen of residents aged ¡Ý35 years old was collected . Chi-square test, t-test and logistic regression analyses were then performed.
Results: The total prevalence of prehypertension was 36.34%. The prevalence of prehypertension was higher in males (39.50%) than females (33.41%) (x2=58.989, P<0.0001), prehypertension prevalence decreaded with increasing age in male, female, and combined groups. Multivariable logistic regression showed that being male (odd ratioo (OR)=1.733), having a family history of hypertension (OR=1.346), overweight (OR=1.332) and obesity (OR=1.537) and diabetes (OR=1.691) were risk factors for prehypertension. The risks for prehypertension in people aged ¡Ý45 years were increased, and the corresponding OR were 1.459£¬1.846£¬2.449£¬2.421£¬3.998, respectively. High-density lipoprotein cholesterol was shown as a protective factor (OR=0.794). Conclusion: The prevalence of prehypertension was very common in rural areas, and the prevalence was high in teenagers. Thus prehypertension prevention should be begun with teenagers. The risks for prehypertension increased for residents aged ¡Ý35 years. Frequent BP monitoring was needed as well as controlling of BP by non-drug methods.

Maternal Health Seeking Behavior in Rural Nigeria (26 Nov 2007) Maternal mortality has been known to be associated with three phases of delay in utilizing modern health care facilities. Phase I delay is largely due to delay in decision-making. This study was carried to find out what extent women in the reproductive age group can take decisions in matters affecting their health. 180 rural women of the reproductive age group were interviewed with the aid of a questionnaire in order to determine the significant persons who make health-seeking decisions for them. They were also asked to indicate their preferred health care facilities. It was found that 75% of rural women in the community studied are able to make their own decisions on matters concerning health. Younger women especially adolescents were likely to depend on decisions made by parents while older and less educated married women were more likely to depend on their husband's decision. Ninety four percent of the women indicated preferences for modern health care facilities among alternative choices. It is therefore suggested that parents should be involved in programs designed for adolescents while husbands should be involved in programs deigned for older and less educated women. It also suggested that educational and financial empowerment of the girl child can minimize phase I delay and reduce phase I dependent maternal mortality.
